Qatar’s Ministry of Public Health mandated the quadrivalent meningococcal ACYW-135 vaccine for every pilgrim traveling to perform Hajj 1447H/2026 while requiring COVID-19 immunization for those at elevated risk of complications, the ministry announced on April 22, 2026. Primary healthcare centers stand ready to administer the shots, which pilgrims must receive at least 10 days before departure to Saudi Arabia to meet both Qatari and Saudi entry standards. The directive further recommends seasonal influenza vaccination for all pilgrims aged 18 and above along with optional pneumococcal and respiratory syncytial virus vaccines for older adults and those with underlying conditions.

The ministry’s April announcement listed specific high-risk categories that must obtain the COVID-19 vaccine, including pilgrims older than 65, pregnant women and individuals suffering from chronic heart or respiratory disease, kidney failure, hereditary blood disorders such as sickle cell anemia or thalassemia, immunodeficiency, cancer or chronic neurological conditions. It advised all other adults to receive the COVID-19 vaccine voluntarily while urging everyone to secure the influenza shot before travel. The ministry also made the pneumococcal vaccine available to those 65 and older or aged 18 to 64 with risk factors ranging from diabetes and smoking to splenectomy, according to its published guidance.

Saudi Arabia’s Ministry of Health requires the same quadrivalent meningococcal vaccine for all international pilgrims arriving for the 2026 season, with the dose administered no less than 10 days and no more than five years before arrival in the case of conjugate formulations. A World Health Organization assessment of Hajj health protocols has consistently highlighted meningococcal vaccination as essential to prevent outbreaks that historically affected gatherings in 2000 and 2001 when serogroup W predominated. The alignment between Qatari and Saudi rules ensures seamless compliance for the roughly 4,400 pilgrims allocated to Qatar under the kingdom’s quota system.

General Authority for Statistics figures show that 1,707,301 pilgrims performed Hajj 2026, an increase of 2.04 percent from the previous year, with 1,546,655 arriving from outside Saudi Arabia. The scale of the event amplifies transmission risks through crowding and shared surfaces, a reality the Qatari ministry addressed by publishing a Pilgrim’s Health Guide in cooperation with the Ministry of Awqaf and Islamic Affairs and Qatar Red Crescent. That guide, available on the ministry’s website, supplies preventive advice covering the period before, during and after the pilgrimage.

The Ministry of Public Health made all listed vaccines accessible free of charge at primary healthcare centers across Qatar and stressed the importance of early consultation for pilgrims with chronic illnesses to secure sufficient medication supplies. Its April 2026 circular forms part of broader regional preparations that the Saudi Ministry of Health coordinated with Gulf Cooperation Council partners to standardize health safeguards. Centers for Disease Control and Prevention travel guidance similarly recommends that pilgrims update routine immunizations and consider additional protections against respiratory viruses before attending the annual rites.

Qatar’s measures correspond with the National Health Strategy 2024-2030, which prioritizes proactive vaccination to lower disease burden and support population wellbeing. The ministry’s focus on high-risk groups reflects data from previous Hajj seasons showing higher complication rates among older pilgrims and those with comorbidities. Health authorities continue to monitor global infectious disease trends that could influence future requirements for the world’s largest annual gathering.
